What to bring to your first appointment
Please try to arrive ten minutes before your appointment so you’ll have time to go through our intake process.

- A list of medications, allergies, and medical conditions
- Your health insurance card and booklet if you have one
Financial policy
Payment is due at the time of treatment. If you need to make payment arrangements, please speak to our front desk staff before your appointment.
Insurance
During the checkout process we will submit your dental claims for you to ensure speedy reimbursement. Your insurer has updated privacy rules and, unfortunately, we are no longer able to call on your behalf to verify eligibility. Please note that you are responsible for your treatment costs. This includes any costs not covered by your insurer.
